Rarely do accidents happen without warning. Most frequently, people are aware of a situation that is hazardous but fail to report it until tragedy has struck. If you are witness to a "near-miss" or see any process or activity that you feel may result in an accident, please report it immediately. Please be aware of any potential incident anywhere in your workplace from parking your car to working on set. We all need to work together to keep each other safe so please submit any concerns you have, regardless of department of production.

All submissions are kept strictly confidential and will be forwarded to the IATSE Safety Representative for action. This will protect you and your crew's identity.
